Does sanitizing actually fix a duct odor?

Usually not by itself. Sanitizer sprayed over loose dust just coats the dust. Mechanical cleaning comes first, every time. Then an EPA-registered antimicrobial labeled for HVAC interior surfaces, applied with the dwell time the label states. Ask to see that label. If nobody can show you one, skip the step.

What your ducts are made of decides everything. Original galvanized sheet metal trunk-and-branch in postwar ranch homes takes contact cleaning and treatment well. Mid-century wrapped fiberglass ductboard does not. Porous liner holds odor inside the fiber. Once that liner is deteriorated and shedding, sanitizing is cosmetic and replacement is the honest answer. R-6 and R-8 flex has a plastic inner core that treats fine, but a kinked, crushed or chewed run still gets swapped.

Rodent entry and nesting is the common driver here, especially on wooded and hillside parcels. Urine odor lives in nest material and attic insulation, not the airstream. Nesting out, entry points sealed, then treatment. Reverse that order and the smell returns.

Some jobs need no sanitizer at all. Construction drywall dust after a remodel and fine wildfire ash in returns are particulate problems. Cleaning plus a filter rack that actually seals solves those. Condensation on cold supply runs will restart growth unless the insulation gets corrected.
